In this bill of lading, you will see the Name of the Company at the upper left part with the Address, Telephone and Email address below. On the opposite side are the Trip # and Order #. The first part of the form is portioned in two columns, the ORIGIN and the DESTINATION, where both have the same information to fill in, the Customer Name, Address, City, ST, ZIP and Phone. Below, are the Year, Make, Model, Plate, Odom VIN, and the Interior Condition. Then, you have to specify the possible conditions during the inspection and a pick a box if necessary; choose either Dark, Rain, Snow, Dirty and Photos taken with a Digital Camera.
The next thing you will see are the illustrations of the vehicles, on both front and back. In a small space in the right side (below the illustrations), there is a “Vehicle condition at origin:” with the signature of the driver, COD, Biliing and the Amount. While the bigger space on its left part is the Chart of Identification Codes, initially these are the physical conditions of the vehicle that has been seen during the inspection.
The last part is divided into two, first is the Origin Notes with the Customer’s and Driver’s Signature. And, on the other side is the Destination Notes together with the Signature of both the Customer and the Driver.
